What is a Medical Treatment Decision Maker document or AMTDM?

It is a legal document with similar powers to the Power of Attorney document which specifies who has the authority to make medical decisions on your behalf when you are not able to. This person or persons will have the authority to consent to or refuse medical treatment based on your preferences and best interests. The new AMTDM replaces the previous Enduring Power of Attorney (Medical Treatment) document.
